Please fill each form out completely - both front and back - and be sure to sign and date each page. It will help us greatly if you remember to use BLACK INK ONLY. Thank you!
Please include all medications that you take and any medication or other allergies that you have.
When you come in for your first visit please bring
-- copies of these forms,
-- your insurance cards,
-- referral authorization (if applicable), and
-- picture identification.
Also, please bring all available medical records, testing reports, or films that pertain to your condition.
Please do not mail or fax paperwork back to us, but bring it with you to your appointment.
We need all of these items to see you at your scheduled appointment. Thank you for understanding.
